How do I grant or remove Power of Attorney for an account?

Power of Attorney can be granted or removed for an Optimum Mobile account. You can follow the directions below to complete a Power of Attorney request.

Steps to grant or remove Power of Attorney:

  • Download the Power of Attorney form
  • Fill out the form. All fields are required unless indicated as optional.
  • Provide copies of legal documentation granting or disallowing Power of Attorney
  • For the person being granted Power of Attorney, including a copy of a government-issued photo ID
    • Driver’s License
    • Military ID
    • Passport
    • Permanent Resident Card
  • Submit the form and all supporting documents to:
